The Work Tax Team

Complexity
of the work

S-corps, K-1s, multi-state. The good part of the job.

Doc Wealth serves physicians and their practices, and physician tax situations are rarely simple. A typical client has an S-corp, a backdoor Roth, retirement plans stacked three deep, and income crossing state lines. We've automated the mechanical parts of the job so our tax team spends its time where judgment actually matters: planning, advising, and getting hard calls right.

One client, anatomized

Dr. K.

41, anesthesiologist. W-2 at a hospital system, 1099 locums on the side, and a telehealth panel she picked up last spring.

0 Moving parts on one return
Composite client. Details changed.
  1. 01
S-corp on the locums income Reasonable owner comp, a W-2 out of her own entity, and a K-1 that has to agree with both.
  2. 02
Hospital W-2 Withholding that has to be trued up against everything the S-corp is about to do.
  3. 03
Solo 401(k) The employee deferral is already spent in the hospital plan. Only the employer side is left.
  4. 04
Cash balance plan on top Actuarial contribution, sized off the comp number you set back in line 01.
  5. 05
Backdoor Roth Pro-rata rule against the rollover IRA she has been sitting on since residency.
  6. 06
PTET election A credit worth real money, and only if the payment lands in the right year.
  7. 07
Three states Resident state, locums state, telehealth patients in a third. Credits for taxes paid, in the right order.
  8. They all touch each other. Move the owner comp in line 01 and four of the others move with it. Software will not tell you which way.
Advising, not entering

Where the automation stops

Nobody here spends their day keying W-2s into software. Document intake, tie-outs, and first-pass review are handled by our internal tooling. Your job starts where the automation stops: interpreting the numbers, spotting what's off, and telling the client what to do next.

Genuinely technical clients

Decisions, not forms

Our clients are physicians with layered situations: S-corp owner compensation tied to K-1s, backdoor and mega backdoor Roths, cash balance plans on top of 401(k)s, PTET elections, and multi-state filings from locum and telehealth work. Every return is a set of decisions, not a form to fill.

You lead the call

Clients don't hire us to file. They hire us to plan. Entity structure, compensation strategy, retirement plan design, and state exposure are conversations you'll be leading, not passing up the chain. You'll see the advice land in real dollars for real people.

You'll get better here, fast

Complex work compounds

A year of physician S-corps, PTET credits, and multi-state returns teaches you more than five years of standard 1040 volume. We review deeply, explain our reasoning, and expect you to push back when you disagree.

You'll hate it here if
  • You want volume and a checklist.
  • You'd rather route the hard call up than make it.
  • You measure the season by returns filed.
You'll like it here if
  • You want to be the one who decides.
  • You'd rather be told your reasoning is wrong than told nothing.
  • You want the physician on the phone, not their file in a queue.

It's harder than the average tax job. It's also the reason people stay.
